Cards are optimized for printing double sided in colour, in a 4x6 inch size. Many home printers can print on 4x6 inch cardstock (use the borderless setting if available). The cards will be most resilient if they are laminated. Heavy cardstock with a glossy finish, similar to ACLS cards, is also a good choice, though it may not withstand wiping for cleanliness.
Hole punch the top left corner of the cards, ideally with a 1/8 inch hole punch, and use a binder ring to hold the set together. A coil ring binding along the left margin may also work, though you risk losing content.
If you are considering professional printing (+/- lamination), let the print shop know that they can trim up to 1/8 inch from all sides of the cards if needed. Any local print shop should be able to do this work for you.

Updated DKA and HHS in Adults (O2 Reverse) - Added recommendation to consider giving a long acting basal insulin such as glargine in patient's usual 24 hour total basal insulin dose (or 0.25 units/kg of glargine if insulin naive) at the same time as starting the traditional insulin infusion. This has been shown in retrospective observational studies to lower the required dose and shorten the duration of insulin infusion, lower the incidence of rebound hyperglycemia, result in faster resolution of acidosis, and shorten the hospital stay (Cardoso et al, see References)
